We will collect information you provide to us such as: your name, address, phone number, e-mail address, date of birth, your debit or credit card details (We do not store your credit card details) and information about your health. We may collect any communications between you and us, as well as any additional information you provide.
We will collect personal information about you directly from you through phone or email; where you have submitted a Bioscreen request or provided us with samples; or filled in the Bioscreen Patient Registration Form. We also collect personal information about you from your practitioner or parent/ guardian when they submit a Bioscreen request on your behalf. If we are unable to collect some or all the personal information we require, we cannot provide you with our services that you are requesting.
